Payroll Set-Up

To get started with my payroll service please download the following set-up forms and return them to me via email or fax:

• Engagement Letter
• Employer Information Form
• Employee Information Form (please complete one for each employee)
• Contractor Information Form (please complete one for each contractor)
• Direct Deposit Authorization Form (please complete one for each employee and each contractor so their pay can be directly deposited into their bank account)
• Direct Withdrawal Authorization form (please complete for the business - so your taxes and employee and contractor pay can be withdrawn from your business account)
• Form W-4 (to be completed by all employees to determine the amount of Federal withholding)
• Form W-11 (to be completed by HIRE credit employees)
• Form I-9 (to be completed by all employees and contractors)
